Why do you want to be part of the staff team? I would like to be part of the staff team to have events to bring enjoyment to the people that play on the server. I also would like to help remove the minge problem that happens at night. I also find bugs with things that can be spawned in and I would like to help with them. Helping with the issues people cause will be my number one priority though.
Why should you be accepted on the staff team? I should be accepted so I can help solve problems that people cause. I usually don't see admins on during CDT Night Time, though I am almost always on at those times, the times when the minges come out, and I could deal with them. I am usually on 9 A.M. to 10-12 P.M.
Do you understand that you must change your Steam name to "[DEFN] (Username)" upon getting accepted? Yes I do.

An MPF Union kills a Civilian but you don't know the reasoning behind the killing.
What do you do? I would pull the the Civilian and MPF Union to a sit and ask what was going on, then for both sides of the story.
An H.E.C.U. Soldier enters the Combine base to "RP defect".
How do you proceed as a staff member? I would pull the H.E.C.U. Soldier to a sit and ask him what he was doing. Depending on what they had done, I would give a verbal or an actual warning.
You find a SU/MACE CO infiltrating without notifying their team in chat. You bring them into a sit to talk to them about breaking the rules. In this sit the CO threatens to demote your RP character if you don't let them go.
How do you handle this? I would bring it up with their Higher Ups and see what they would do about it as well as giving them a warning for not notifying their team.
A group of players comes onto the server and threatens to DDOS or raid it.
What do you think you should do? If they would threated to DDOS I would IP Perma Ban them because that is how I have seen them dealt with before. If they threatened to raid I would kick them and if they come back I would ban them for a few days.
You see a high ranking staff member doing something questionable, but you don't know if it is allowed.
What do you do? I would go question them about it and contact a higher ranking staff member to come check it out.

Answers are a bit too shallow, didn't answer the "Previous bans?" question, and you didn't properly give all reasons of the warn (Which i think is important to hear about first).
I think this application could be improved by going into more detail about what you would do and giving more and proper information, also kinda hard to read through the text when it is all in bold.
