I recall it being unfeasible due to technological limitations. Although I wouldn't say an underslung 40mm with buckshot would have anywhere the same potency as the M680 currently does, even with the versatility of being able to shoot actual grenades in addition to whatever ammo options it would have available.
ADDENDUM: It'd also be a nightmare, trying to push a marine and not knowing if he was ADS'ing with his rifle or the 157 caliber underslung shotgun.. or grenade about to embed itself in your face.
From what i have seen, the MWB devs are working on adding underbarrel shotguns and grenade launchers, and in the vanilla MWB you can see the players exact weapon and attachments (even the laser) but for the server it was replaced with CSS weapons to save disk space and not use CPU and GPU resources, aka for better performance
For someone who has a beefy cpu this would not be a big problem, but I can see it as a problem for people with lower end pc's

Oh believe me it would be, gmod don't give no shits nuh uh
Generally speaking its balanced as it follows its own niche and playstyle. It follows a preexisting concept that used to follow Phantom back in the day when it existed with a AR and pump shotgun as well as misc equipment such as grenades.
Personally I think its fine where it is, its not a hard class to combat, its a typical shock-infantry class that is meant to play aggressive while plugging holes in attacks or defenses (or at least that's how I interpret it).

i know, however the Source 1 engine is more CPU reliant unlike Source 2 which is GPU reliant on perfomance
